Just click on this link and fill out your prescriptions with the name, dosage (mgs) and frequency (how many per day). Then fill out the Scope document giving us permission to speak to you about the results. We will review and give you a call with the results. For the upcoming AEP (Annual Enrollment Period) from Oct 15th – Dec 7th, please complete this as soon as possible, preferably in Sept.
